In-town Requests
Transcripts and records from 2007 through present and currently enrolled students may be obtained from:

Transcripts and records from all other years may be obtained from:

• Print and complete the NISD Transcript Request Form.

• Transcripts may not be faxed without written permission. 

• Transcripts include grades, test scores, shot records, and SAT and/or ACT results.

• Copies of Diplomas are NOT kept on file.

Please bring your Driver's License for signature verification when you pick up your transcript.

Out-of-Town Requests
If out-of-town, the student will need to send a signed release with the following information, or complete the NISD Transcript Request Form.

1. Year of Graduation or Withdrawal

2. Your name as it appears on your transcript (include maiden name)

3. Social Security number

4. Date of Birth

5. Copy of your Driver's License for signature verification

6. Stamped self-addressed envelope
